  • If applicable, responses to the program’s appeal are addressed, and a follow-up Licensure Review may be scheduled.

It’s Time to Pay the Big Bucks

  • The program submits a $1,500.00 nonrefundable licensure review fee (for first-time applicants only) after the review.
  • The formal license is issued only upon receipt of payment of the licensure review fee.

A Complete Addiction Treatment Application Must Include:

  1. Governing body information
  2. Program planning and evaluation plan
  3. Employee handbook
  4. List and description of program services
  5. Proof of physical inspection
