  2. Notarized application form
  3. Proof of zoning approval from city and/or county, if applicable

  7. Three years of business history and/or resumes, along with three letters of reference for each corporate officer

  11. Criminal background checks for all relevant parties
  12. Floor plan with dimensions
  13. Medical labs attestation

  16. An 8” x 11” photograph of the existing facility or an estimated date of completed construction for new buildings
